How do I schedule an appointment through MOMS?

If you have a test kit from a MOMS Partner Lab/Client, you can schedule your appointment in a few different ways.

When a patient has a test kit from a MOMS Partner Lab/Client, the next step is to schedule their collection appointment.

This can be done a few different ways: via the MOMS Website, or via a Custom URL/Custom QR Code tear sheet.

MOMS Website

If scheduling via the MOMS website, click "Schedule Collection," as seen below.Screenshot 2024-05-16 at 1.44.49 PM

This will take you to the MOMS Patient Portal. The first step is to select the Lab/Client the kit is from.

Screenshot 2024-05-16 at 1.58.04 PMEnter your zip code to find a MAP: Medical Access Point/Provider™ at which to schedule the required collection.

If the patient is responsible for the draw fee, valid credit card information is required at the time of scheduling. Please note, the credit card is not charged until your appointment is completed.

Once your appointment is scheduled, you will see an Appointment Confirmed page with relevant appointment instructions and information. Patients will also receive a confirmation email with this information. The night before the appointment, patients will also receive a reminder for their upcoming appointment.

Custom URL/QR Code

For Labs/Clients looking to automate the patient scheduling process, a patient can click a Custom URL to direct them to the third stage of the patient journey - entering their zip code. This URL can be provided to a patient via email or on the Lab/Client's website.

Additionally, Labs/Clients may opt to include a Custom QR Code Tear Sheet in their test kit (example below). When scanned, this will take the patient to the third stage of the patient journey as well - the zip code entering stage.
